Athletic Physical Form  OR  Alternate Year Card 

REQUIRED each school year for students wishing to participate in any MS Athletics. Middle school students participating in athletics will need a physical form OR alternate year card on file prior to the first day of practice.  Doctor-signed athletic physicals are valid for two school years if done after April 1.
